Introduction

The Tailored Care Model (TCM) aims at strengthening the mental health condition of people using a coordinated system of care that is designed to address both medical and non-medical issues appropriately.Individuals enrolled in Tailored Care Management will be assigned a Care Manager who assists and guides them through their treatment. They will also coordinate with the physicians and other resources involved in the treatment process. A Tailored Care Manager is skilled in supporting individuals with mental health concerns, developmental disabilities, traumatic brain accidents, and substance abuse disorders.

What is Tailored Care Model (TCM)?

The Tailored Care Model program focuses on designing a well-defined process that includes: Your designated Care Manager guides you in various aspects of life that may affect your mental health and well-being, such as providing assistance and support for housing, employment, and food security-related needs of the individual. They create a plan to support your medical needs, behavioral health, and physical health needs. Additionally, the Care Manager will further extend personalized support and guide your family members or caregivers with careful management of your mental health needs. They serve as a dedicated point of contact while collaborating for your specialized care.

The UCS Tailored Care Model program works on the below-mentioned areas:

  • Examine the Present State: Analyzing the present care model and finding any missing links in it to improve mental health, basic Maslow needs, crisis needs, medication management, peer support, and client-centered approach. Also, understand the engagement of the multidisciplinary team.
  • Creating Clear Goals: Appropriately defining the expected outcome of the tailored care model based on the different areas of mental health assistance.
  • Finding Resources: Identifying the required resources for training programs, infrastructure, staffing, and technology to be used for the effective execution of the care program.
  • Preparing Multidisciplinary Team: Prepare a diversified team of psychologists, mental health experts, peer support staff, nurses, social workers, and others required for the TCM program.
  • Create Care Steps: Prepare clear pathways to outline the process in an ordered and organized format to handle mental health concerns.
  • Execute Training Programs: Proper training and guidance are offered to the entire team to enhance their knowledge and skills to tackle mental health crises, medication management, and proper use of peer support.
  • Improve Collaboration & Communication: Ascertain smooth communication and coordination among different co-members active in offering care to the patient. Organize frequent team meetings and review sessions to understand improvements, and hurdles, and share knowledge to see positive developments in the patient.
  • Unify Peer Support Workers: Bring in peer support workers; who have proper experience in handling mental health concerns in the care model. They can support and assist well through their experience, be empathetic, and understand patients in both inpatient and outpatient settings.
